Summary:
The Diabetes Educator collaborates with other members of the healthcare team, patients, and families to consult on and facilitate diabetes education within the organization and the community; focuses on consultations for patients that are newly diagnosed and/or have complex diabetes management needs; makes decisions guided by evidence based best practices, as well as policies, procedures, guidelines and standards; provides nutrition services to a variety of specialty clinics as well as outpatients by referral from healthcare providers; implements best practices consistent with the principles and values put forth by the Academy of Nutrition and Dietetics; acts as a team member with other healthcare professionals working to advance the health of each patient and the community through optimal lifestyle practices; works collaboratively with other clinical staff to meet patient and family need (i.e., a multidisciplinary approach to care).
Responsibilities:
Delivers diabetes education and instruction using evidence based practice; educates patients on the necessary lifestyle changes involved in a diabetes diagnosis, including nutrition, medication, physical activity and glucose testing; provides consultation for the care of diabetic patients utilizing evidence based practice; applies appropriate standards (AACE, ADA, Endocrine Society, etc.); assesses learning/self-care behaviors; assesses medical/health/psychosocial and economic status implications; assesses current knowledge and self-management skills; collaborates with patient/family/care giver/healthcare team to develop individualized diabetes education plans, instructional methods, and behavioral goals; teaches/counsels regarding principles of care: Baseline knowledge concepts related to diabetes, metabolic monitoring; nutrition principles and guidelines; physical activity; pharmacological management; acute, chronic complications and comorbidities: causes, prevention and treatment; other self-management issues.
Collaborates and delivers evaluation of the Diabetes Education; collects data through computer system and databases; shares program outcomes and results on a quarterly basis; identifies and recommend opportunities for diabetes education improvement; identifies and communicates trends in diabetic patient populations; engages in diabetes advocacy with community awareness, health fairs, media, etc.
Performs activities that create a compassionate, supportive, and therapeutic environment for patients and healthcare staff, with the aim of promoting comfort and healing and preventing unnecessary suffering. Includes, but is not limited to vigilance, engagement, and responsiveness of caregivers, including family and healthcare personnel; listens to patients and families ; individualizes care and fosters therapeutic relationships; approaches patients and families with compassion, and offers emotional support; practices reflect recognition of the impact of family on patient outcomes; demonstrates understanding of the National Patient Safety Goals and impact to patient care; communicates effectively with peers, physicians, hospital departments, and other hospital resources to effectively manage patient care; identifies obstacles or barriers to communicating effectively with patients, families, and healthcare team.
Works with others in a way that promotes/encourages each person's contributions toward achieving optimal, realistic patient/family goals; involves intra and interdisciplinary work with colleagues and community; recognizes need for interdisciplinary contributions to the patient plan of care; values collective contributions of interdisciplinary team members; utilizes standard reporting system (SBAR) to effectively communicate patient care needs; demonstrates support, validation, and respect for team members to establish a foundation to care compassionately for patients and families; demonstrates clinical competence, confidence, and clarity in communications with co-workers; makes appropriate referrals and follow up.
Provides evidence-based quality nutrition services to an outpatient population of varied age and wellness; focuses on the collaborative management of chronic disease (i.e., oncology, obesity, CHF, hypertension, diabetes, COPD, geriatrics etc.); utilizes relevant data and information to identify nutrition-related problems in the continuum of care; encourages frank, two-way communication throughout the organization;
Listens to current patients and stakeholders to foster improved nutritional services; works in an environment of organizational performance improvement focused on achieving strategic program objectives and goals; develops and implement nutritional programs to meet service line and community need (evidence-based programs that are efficient and well-coordinated);
Identifies and implements appropriate, purposefully planned actions designed with the intent of changing a nutrition-related behavior, risk factor, environmental condition, or aspect of health status for an individual, target group, or the community at large; monitors and evaluates indicators and outcomes data directly related to the nutrition diagnosis, goals, and intervention strategies to determine the progress made in achieving desired outcomes of nutrition care and whether planned interventions should be continued or revised.
Minimum Requirements
- High School Diploma or GED
- Basic Life Support (BLS)
- Registered Dietitian (RD)
- Certified Diabetes Care and Education Specialist (CDCES)
- 2+ Years of related experience
