Summary:
A Nurse Extern II, under the direction of an RN, performs skills for which he/she has received didactic instruction in his/her program up to, but not including, medication administration and IV therapy.
Responsibilities:

Under the supervision of a registered nurse, works on another’s behalf and representing the concerns of the patient/family and nursing staff; serving as a moral agent in identifying and helping to resolve ethical and clinical concerns within and outside the clinical setting; Identifies the client/family’s need for basic information in a clear, understandable language as evidenced by documentation and team feedback; Demonstrates and understands the concept and importance of client/family advocacy; Maintains and is respectful of client confidentiality.

Under the supervision of a registered nurse, develops the sensitivity to recognize, appreciate and incorporate differences into the provision of care. Clinical Inquiry:
The ongoing process of questioning and evaluating practice and providing informed practice. Under the supervision of a registered nurse, creates practice changes through research utilization and experiential learning; Reads and follows nursing standards and organizational policies and procedures; Appropriately uses the nursing process; Asks for personal feedback on clinical practice as evidenced by preceptor/team feedback; Evaluates nursing care of clients as evidenced by documentation.
Clinical Judgment:
Under the supervision of a registered nurse, demonstrates clinical reasoning, which includes clinical decision making, critical thinking, and a global grasp on the situation, coupled with nursing skills by a nurse through a process of integrating formal and experiential knowledge; Demonstrates commitment to patients by participation in the implementation of the nursing process in providing patient care, initially under the direction of the RN preceptor and/or Resource Coordinator, and in simulation situations; Seeks advice for situations not familiar to them; Demonstrates ability to prioritize, seek appropriate resources when needed and use of evidenced based practice as applicable for situation.
Caring Practice
Under the supervision of a registered nurse, demonstrates nursing activities that create a compassionate, supportive, and therapeutic environment for patients and staff, with the aim of promoting comfort and healing and preventing unnecessary suffering. Includes, but is not limited to, vigilance, engagement, and responsiveness of caregivers, including family and healthcare personnel; Initially performs assessments focusing on the physical and psychosocial needs of the patient in an ongoing and systematic manner as evidenced by documentation as evidenced by team feedback and in simulation situations; Participates with preceptor in formulating an outcome-based plan of care which is prioritized and can be based on nursing assessment; Care based on standards and protocols; Maintains a safe physical environment; Acknowledges patient’s/families need for comfort and concerns; Demonstrates compassion and an interest for clinical inquiry; Acknowledges death as a potential outcome.

(Safety / Quality Improvement)
Under the supervision of a registered nurse, develops a body of knowledge and tools that allow the nurse to manage whatever environmental and system resources exist for the patient/family and staff, within or across healthcare and non-healthcare systems; Utilizes the nursing process to compare patient response with observable outcomes specified in the plan of care as evidenced by documentation and in simulation situations; Demonstrates and verbalizes understanding of the concept of implementing care and eventually applies and evaluates application of this concept in an effective manner, as evidenced by preceptor, team, and/or patient feedback and in simulation situations; Progresses from viewing the client as a "task" to viewing the client holistically; Maintains and updates skills checklist and provides updated list to Resource Coordinators and/or unit educator/director as designated.

Under the supervision of a registered nurse, works with others in a way that promotes/encourages each person’s contributions toward achieving optimal, realistic patient/family goals. Involves intra and interdisciplinary work with colleagues and community;
- Communicates courteously and effectively respecting confidentiality requirements. Demonstrates ability to work in a team environment. Maintains ongoing communication with unit educator, director and preceptor , coach or mentor for ongoing evaluation of practice. Actively listens to patient/family to ensure appropriate communication and accurate relay of information Involves patient/family in decisions related to healthcare needs.
- Implements care in a knowledgeable and skillful manner, as evidenced by team feedback and documentation, and patient feedback, and in simulation situations
- Initially, evaluates effectiveness of care given by self with the ability to eventually evaluate both the effectiveness of care given by self and other health team members, in progressing patient toward outcomes

Facilitation of Learning
Under the supervision of a registered nurse, demonstrates the ability to facilitate learning for patient/families, nursing staff, other members of the healthcare team, and community. Includes both formal and informal facilitation of learning.
- Participates in identifying patient/family/significant other's education needs and in simulation situations.
- Collaborates in the identification of discharge planning issues based upon admission assessment, and works with their team to makes appropriate referrals and monitor patient/family/significant other's progress and readiness for discharge.
- Progresses from viewing client education as a systematic task to evaluating the clients’ readiness to learn and understand the information as evidenced through documentation and preceptor/team feedback.
- Sets individual (self) goals to accomplish within a specified period of time.

REQUIRED: Graduate within the last year from an accredited School of Nursing. Nurse externs have fulfilled all requirements to qualify to take the licensing exam.
